Providing more holistic care to identified at-risk servicemen and women, while also transitioning to a new electronic health record (EHR) system, is no small challenge for the Department of Veterans Affairs (VA). Additionally, moving to a brand-new system of record must not disrupt current patient engagement and care.
New and innovative CDS technologies that offer holistic care dashboards can help aid this by presenting patient data in real-time. All the “dots” get connected, including the linkage of outpatient care with in-patient. Also, for the first time, this total picture can include socio-economic data, such as lack of transportation that could affect accessibility to care.
